Fighter Participation Agreement

Section 2

Additional Participation Requirements

Fighters must report to the venue at least three hours before the scheduled event start time for check-in, weigh-ins, fighter experience activities, rules review, equipment inspection, and any required pre-bout medical review.

Required documentation due at least three weeks before the event Proof of required medical testing. Valid government-issued national photo identification. Current physical examination or medical clearance. Any additional documentation required by the sanctioning body, venue, insurer, or applicable law.

Equipment and compliance Approved shorts with no pockets. Approved mouthguard before entering the competition area. Approved male groin protector/cup, if applicable. Equipment is subject to inspection and approval before the bout.

Section 3

Liability Waiver and Assumption of Risk

The Fighter acknowledges that participation in mixed martial arts involves inherent risks, including serious bodily injury, permanent disability, and death. Participation is voluntary.

The Fighter grants Hoosier Havoc LLC permission to photograph, record, livestream, and otherwise capture the Fighter's name, voice, likeness, image, and performance during activities associated with this event for lawful promotional, educational, archival, documentary, advertising, marketing, and social media purposes.

Fighter Code of Conduct

Required conduct *

Treat officials, opponents, coaches, volunteers, vendors, and spectators respectfully. Follow instructions from event officials. Avoid threatening or abusive behavior. Refrain from fighting outside authorized competition. Avoid damaging venue or Company property. Maintain professional conduct before, during, and after the event.
